FAFSA or DREAM ACT APPLICATION VERIFICATION & WAIVER FORM

The Free Application for Federal Student Aid (FAFSA) or Jose Peralta New York State DREAM Act (DREAM Act) determines a student’s eligibility for financial aid to assist with college or career school attendance.

Under New York State Education Law §305(61), school districts must verify that high school seniors have either:

  1. Completed, or plan to complete, the FAFSA or DREAM Act application, or

  2. Signed a waiver indicating that they understand what these applications are and have chosen not to complete one.

Please complete the electronic form or print the attached form and return it to your high school counselor by the by FRIDAY, JUNE 5th.

Completion of this form is for verification purposes only. The student and/or parent/guardian will not be penalized or prevented from graduating for declining to complete the FAFSA or DREAM Act application or this waiver.
