IMPORTANT: MILITARY OPT-OUT

Dear Parent/Guardian,

In accordance with the "No Child Left Behind Act" of 2001 and the Elementary and Secondary Act, high schools are required to release directory information (students name, address and phone number) of 11th and 12th graders to military recruiters upon request.  However, parents have the right to refuse to have such information released.

Please indicate your preference and electronically sign the form below.  Please note, a separate form will need to be completed for each of your 11th and 12th grade children.  If we do not receive the form by October 10, 2025, the law presumes that you are allowing the release of this information, and we will include your child's name in the directory of information given to the military recruiters.

Please complete the parent square form or print and sign the attached form and return it the high school counseling office.

CLARIFICATION ON HOW TO ANSWER THE QUESTIONS ON THE ELECTRONIC FORM:

If you DO NOT want your child's information shared with the Military, you would check "NO" to the first question, and "YES" on the second question, which indicates you DO NOT want the information released.

If you DO want your child's information shared with the Military, you would check "YES" to the first question, which indicates that you DO want the information released, and "NO" on the second question.
